Indications for taking Xeljanz 5mg.
– treatment of moderately severe and severe active rheumatoid arthritis in adult patients with insufficient response to methotrexate or in case of intolerance (as monotherapy, as well as in combination with methotrexate or other nonbiological disease-modifying anti-rheumatic drugs (DMARD, basic remedies)).
It is not recommended to apply Xeljanz 5mg in combination with biological disease-modifying antirheumatic remedies or with powerful immunosuppressants such as azathioprine and cyclosporine.
Dosing regimen.
The remedy is taken orally regardless of the meal.
The recommended dose of the remedy for rheumatoid arthritis is 5 mg 2 times / day.
The preparation can be used as a monotherapy, as well as in combination with methotrexate or other basic remedies.
Correction of the dose of the remedy in case of severe infection and cytopenia.
It is not recommended to start therapy with Xeljanz 5mg in patients with an absolute number of lymphocytes less than 0.5 × 109 / L, an absolute neutrophil count of less than 1.0 × 109 / L, or a hemoglobin level of less than 90 g / L.
When developing lymphopenia, neutropenia and anemia, it is recommended to stop taking the remedy.
With the development of a serious infection, you should stop taking the remedy Xeljanz 5mg until control of the infection is established.
The dosage of the preparation should be reduced to 5 mg 1 time / day:
– in patients receiving potent inhibitors of CYP3A4 (eg, ketoconazole);
– in patients simultaneously receiving one or more remedies capable of moderately inhibiting CYP3A4 and strongly inhibiting CYP2C19 (eg, fluconazole).
Simultaneous use of powerful inducers CYP3A4 (for example, rifampin) and the remedy Xeljanz 5mg can lead to a decrease or loss of clinical effectiveness of the latter. Simultaneous application of the preparation of Xeljanz 5mg and powerful inducers of CYP3A4 is not recommended.
Correction of the dose of the remedy in patients with renal or hepatic insufficiency.
The dosage of the preparation should be reduced to 5 mg 1 time / day:
– in patients with moderate or severe renal insufficiency;
– in patients with moderate impairment of liver function.
