Pharmacological action of Docetaxel 160mg.

An antineoplastic agent, a semi-synthetic analogue of paclitaxel. The mechanism of action is associated with the accumulation of tubulin in the microtubules of the mitotic spindle, which leads to disruption in the processes of their assembly and disassembly. It disrupts the cell division in the G2 and M phases of the cell cycle.

Pharmacokinetics of Docetaxel 160mg.

After a single IV injection at a dose of 160 mg / m2, the average Cmax of docetaxel in plasma is 3.7 μg / ml. The average value of Vd in the equilibrium state is 113 liters. Binding to plasma proteins – 90%. Docetaxel is metabolized.
The average system clearance is 21 l / h / m2. About 6% is excreted by the kidneys and 75% by the intestine, mainly in the form of metabolites and, only a small part – unchanged.

Side effects of Docetaxel 160mg.

On the part of the hematopoiesis system: neutropenia, thrombocytopenia, anemia.
On the part of the digestive system: nausea, vomiting, diarrhea, increased levels of transaminases and bilirubin in the serum; rarely – stomatitis.
From the central nervous system and peripheral nervous system: paresthesia, hyperesthesia.
From the musculoskeletal system: arthralgia, myalgia.
Allergic reactions: skin manifestations are possible; rarely – bronchospasm.
Dermatological reactions: skin rash, mainly in the area of the feet, palms, and also in the upper limbs, face, chest, often accompanied by itching, sometimes followed by desquamation; hypo- or hyperpigmentation of nails and onycholysis; alopecia.
From the side of water-electrolyte metabolism: fluid retention in the body, incl. ascites, peripheral edema.
From the side of the cardiovascular system: arterial hypotension, disturbances of the heart rhythm.

Indications of Docetaxel 160mg.

As a means of the first line for breast cancer, as well as ineffective therapy with anthracycline-based remedies; not small cell lung cancer (including with ineffectiveness of other antitumour agents); malignant tumors of the head and neck; ovarian cancer.

Contraindications of Docetaxel 160mg.

Neutropenia, increased sensitivity to docetaxel in the anamnesis, pregnancy, lactation.

Special instructions of Docetaxel 160mg.

Docetaxel is not used when the bilirubin content is more than CAH in combination with the activity of hepatic transaminases, which exceeds the CAH by 1.5 times, and the AF of 2.5 times higher than the CAH.
Before starting therapy, patients are prescribed GCS inside. The likelihood of developing allergic reactions and the occurrence of edema is significantly increased in patients who have not previously been given such therapy. During the period of treatment, a systematic control of the peripheral blood pattern is necessary in order to detect the degree of myelodepression.
